The Chacewater Health Centre is part of a group practice of two full-time and three part-time doctors.  The doctors also work at the Devoran Surgery.  The practice area covers the triangle between Truro, Redruth and Penryn.  We care for patients from all the villages within this area.  We do not register new patients from the towns since these are well served by the town practices.  We aim to provide a full range of family care in a compassionate and professional way.

The doctors employ ancillary staff to work in the practice. There are 3 qualified nurses who undertake the various nursing duties in the surgery. There is a Health Care Assistant/Phlebotomist who performs health checks, supports our nursing staff in chronic disease management and takes blood samples. Mrs. Sue Gunn is the Practice Manager, Mrs. Marilyn Richards is the Dispensary Manager and Mrs. Julia Miles is our data supervisor/IT Manager and also deputises for our Practice Manager in her absence. Mrs Gwen Roberts is the senior member of staff at the Devoran Surgery. There are 8 general receptionists, 4 of whom have a dispensing qualification, 2 of whom undertake most of the secretarial duties and one of whom is the main telephonist for both surgeries.

In addition to the employed staff, Health Authority Community Staff also work from the Health Centre.  We have two Health Visitors, six District Nurses and a District Midwife based with us.  They have their own offices at the Health Centre.  Their contact numbers are listed in the "Useful Numbers" section.

We are a dispensing practice and dispense drugs for patients who live in rural areas further than a mile from a chemist.  Patients may take a prescription to a town chemist if they wish.

  THE DOCTORS and STAFF

All the doctors have completed the Cornwall General Practice Vocational Training  Scheme.  They are married with children..

Dr Mark Gripper qualified at the Middlesex Hospital in 1975 and joined the practice in 1980.

Dr. Russell Bolton qualified at Westminster Hospital in 1982.  He joined the practice in 1987.

Dr Jean-Anne Evers qualified at Edinburgh Medical School in 1974.  She has worked part-time in the practice since 1983.

Dr Tony Shaw
qualified in Liverpool and joined the practice in 2004 as did his wife,
Dr Margie McDermott who also qualified in Liverpool and joined in 2004.

All the doctors are married with families.
 

DOCTORS' LISTS
Patients may see whichever doctor they choose regardless of which doctor's name appears on their registration card.  Whenever possible a continuing problem is best dealt with by the same doctor.

STUDENTS
From time to time medical students are attached to the practice.  You may be asked if they may "sit in" at the consultation.  Please do not hesitate to refuse this request if you do not wish them to be present.  Nobody will be offended.

REGISTERING WITH THE PRACTICE

Our practice area covers the triangle between Redruth, Truro and Penryn. We care for patients from all the villages within this area. We do not register new patients from the towns since these are well served by the town practices. We aim to provide a full range of family care in a compassionate and professional way.

All new patients are required to fill in a GMS1 form (available at reception) and are invited to attend for a “get to know you” appointment with the practice. Patients now register with the practice and are not placed on individual GP lists. However, you have the right to express a preference to receive services from a particular practitioner and this will be recorded in your medical notes.

Whenever possible a continuing problem is best dealt with by the same doctor.

  APPOINTMENTS

An appointment system operates booking patients at ten minute intervals. Appointments may be made at the surgery or by telephone. We would appreciate it if you could observe the following points:

a) If you are unable to keep an appointment, please inform the surgery.
b) Please let the receptionist know the nature and urgency of the problem and they will do their best to accommodate you.
c) We never refuse to see urgent cases. These will normally be fitted in at the end of the surgery.
d) We try to keep to appointment times. However, some problems take longer and sometimes an emergency removes the doctor from surgery. Please be patient.

Dr. Gripper does insurance and driving medicals on a Friday afternoon.

Dr. Bolton does minor operations on Friday afternoons and

Dr. Shaw does minor operations on Thursday afternoons at Devoran

Whichever doctor is the duty doctor does a later surgery on Fridays.

 

The practice values telephone consultations/triage.

 

 

When you make a request to speak to a doctor the receptionist/telephonist will ascertain the urgency of the request. Your details will be taken including name, date of birth, telephone number and a brief explanation of the problem.

If the request is urgent the call will be transferred to the duty doctor. If the request is non-urgent the details are written into the doctors/nurses message book for their attention after surgery.

All telephone consultations/information are recorded in the patient's record.

Services at Chacewater Health Centre

PRESCRIPTIONS

A computer prints the prescriptions and monitors repeat prescriptions. It also prints the labels for the items dispensed. The computers prescription register contains all patients' names to enable us to operate a safe, efficient prescribing system. Please make telephone requests for repeat prescriptions on 01872 562201. This is a dedicated 24 hour line for repeat prescriptions only. It is a simple voice mail message which is easy to use and most patients find very convenient. If you do not like this way of ordering you may leave a written request or a marked repeat prescription form at the surgery. Some patients fax repeat requests.

Repeat prescriptions will be given for one month's treatment at a time. Prescriptions are ready to collect two working days (not including a Saturday or Sunday or bank holidays) after ordering.

The computer system is set to give the amount and frequency of drugs as prescribed by the doctor, usually enough for one month. If prescriptions are ordered early we may need to enquire the reason to ensure that the correct treatment is being given. The normal time to re-order is during the last week of the current prescription allowing the two working days for processing and checking.

HOME VISITS

Please make requests for home visits only when the patient, through illness, is unable to attend surgery. Visits are usually undertaken after the morning surgery unless their nature is of an emergency.

We would appreciate if possible that the request be made early in the morning.

EMERGENCIES

If a medical emergency arises out of hours, or between 1.00 - 2.00.pm, please telephone (01872) 560346 /562200 and your call will be automatically transferred to the answering service.  The out of hours duty is covered by Kernow Urgent Care Services which is a well established system and all doctors working for KUCS are fully qualified GPs.

DISABLED PATIENTS

The health centre has toilet facilities for the disabled and there are no steps to negotiate.  Disabled patients may be brought to the front door by car if necessary.  Please ask for assistance from staff if you have any difficulty.

SPECIAL CLINICS

Incorporated in ordinary surgeries we offer various special services. These include ante-natal checks, various chronic disease checks including diabetic checks, well woman checks and health checks for men, blood pressure checks, ischaemic heart disease monitoring, heart check ups and wart clinics. The doctors also perform minor surgery and joint injections. The practice nurses play an active role in many of these special activities. The doctors are all fully qualified to provide family planning services. Dr. Evers and Dr. McDermott fit coils. The doctors do not undertake home deliveries.

Doctors Gripper, Bolton, Evers and McDermott are all fully qualified to provide family planning services, including the fitting of coils.  The doctors do not undertake home deliveries.

SHARING PATIENT INFORMATION

All staff have an obligation to safeguard the confidentiality of personal information. This is governed by law, their contracts of employment and, professional codes of conduct. Written consent must always be obtained from the patient to divulge any information to a third party. This will be kept in their medical records.

  RESPONSIBILITIES

OUR RESPONSIBILITY TO YOU AS A PATIENT

· You will be greeted courteously
· You have a right to confidentiality
· You have the right to see your medical records subject to the limitation of the law.
· You will be seen the same day if your problem is urgent
· You will be seen by your own doctor whenever possible
· You will be informed if there will be a delay of more than 20 minutes for your appointment
· You will be referred to a consultant when your GP thinks it necessary
· You will be given the result of any test or investigation on request or at your next appointment.
· Your repeat prescription will be ready for collection within two working days of your request
· Your suggestions and comments about the services offered will be considered sympathetically and any complaints dealt with quickly.


YOUR RESPONSIBILITY TO US AS A PATIENT

· Please treat all staff with the same respect we are all just doing our job.
· To attend appointments on time or give the practice adequate notice that they wish to   cancel. Someone else could use your appointment!
· Do not ask for information about anyone other than yourself.
· Tell us of any change of name, address or telephone number, so that our records are accurate.
· Only request an urgent appointment if appropriate. Home visits should only be requested if you are really too ill to attend the surgery, and night    visits should be for emergencies only. Please be punctual, but be prepared to wait if your own consultation is delayed by an unexpected emergency.
· Please allow sufficient time for your consultant's letter or the results of any test to reach us.
· When patients are asked to give two working days notice for repeat prescriptions, please give us this time as it is to allow for accurate prescribing.
· Do let us know whenever you feel we have not met our responsibility to you.
· We would, of course, be pleased to hear when you feel praise is due as well.

COMPLAINTS PROCEDURE

The doctors and staff will try to provide the best treatment for all patients at all times. If anyone has a complaint about the treatment they have received we provide a surgery based complaints procedure.

All complaints will be investigated by the practice manager or the doctor in the practice nominated to deal with complaints. All complaints will be responded to within 5 days and investigated and reported back to the patient within 4 weeks. This procedure is part of the NHS system for dealing with complaints and has been approved by the PCSA. If you feel your complaint has not been satisfactorily dealt with you have the right to complain formally to the PCSA. We can advise you of the procedure should you feel this is best for you.

We welcome any suggestion on how we can improve the services we offer.


VIOLENT OR AGGRESSIVE BEHAVIOUR

Unacceptable behaviour that causes harm or fear of harm to any person within the surgery, its car parking area and grounds, will not be tolerated.

Practice procedures following any incidence of violence or aggressive behaviour will be:-
· To review the incident with the practice partners in order to determine it's severity
· To determine if the patient should be removed from the practice list forthwith.
· To decide if a written warning should be given
· To take no further action as the matter has been sufficiently dealt with by the advice already given.

An on-call telephone service answer our calls between 1.00pm - 2.00pm, if your call is an emergency they will contact the duty doctor.

There is no direct number for patients direct to the Devoran Surgery. Calls are taken at the central practice switchboard in Chacewater. If you need to speak to a doctor or staff member they will telephone you back.
